The Silent Battles Healing Foundation Scholarship and Provider Forms Packet is designed for scholarship applicants, licensed providers, vetted provider partners, and administrative review. Our program connects those who have served and those who have survived with the professional care they deserve.

Who We Serve

SBHF’s scholarship program is open to individuals from four backgrounds, each carrying their own form of invisible burden. If you identify with any of the groups below, you may be eligible to apply.

Veterans

Those who served in the armed forces and carry the weight of military service, combat exposure, and transition trauma.

First Responders

Law enforcement officers, firefighters, paramedics, and emergency personnel who face secondary trauma and cumulative occupational stress.

Victims of Violent Crime

Individuals who have experienced violent crime and are navigating the lasting psychological and emotional impact of that trauma.

Survivors of Sexual Violence

Survivors who are seeking specialized, trauma-informed professional care in a safe and confidential process.

How the Program Works

The Four Pillars of Our Scholarship Program

Our packet is structured around four components that guide every applicant and provider through a clear, accountable process from application to care.

Scholarship Application

A structured form capturing applicant identity, service or survivor background, clinical need, an impact statement, and required documentation.

Medical Necessity Verification

Completed by a licensed provider, this form confirms diagnosis, recommended treatment, and clinical justification for supplementary or medically necessary care.

Vetted Provider Network

Licensed providers apply to join our network by documenting their clinical specializations, modalities, population experience, and administrative agreements.

Disbursement Policy

SBHF issues scholarship funds by direct payment to approved providers only, within fifteen (15) business days of receiving valid documentation, subject to funding availability and internal approval.

How to Apply for a Scholarship

The application process is designed to be straightforward and respectful of your privacy. Below are the steps every applicant moves through from initial inquiry to award.

 Step 1

Gather Your Documentation

Before you begin, collect the required supporting materials:

  • Identity, service, or survivor verification
  • Completed Medical Necessity Form (from your licensed provider)
  • Proof of cost for the recommended care
  • Any optional supporting documents

 Step 2

Complete the Scholarship Application

The application captures your applicant information and service or survivor background. You will select one of the following categories:

  • Veteran (Branch and dates of service)
  • First Responder (Agency and role)
  • Victim of Violent Crime (Date and location of incident)
  • Survivor of Sexual Violence (Optional reference)

 Step 3

Share Your Impact Statement

In your own words, describe your clinical need and answer the question at the heart of our program: “How would this scholarship support your healing journey?” This statement helps our committee understand the human context behind your application.

 Step 4

Provide Consent and Submit

Before submitting, applicants certify the following:

  • The information provided is true
  • Awards depend on funding availability
  • You authorize SBHF committee review of your application

 Step 5

Committee Review and Award Disbursement

Your application is reviewed by the SBHF committee. If approved, scholarship funds are disbursed directly to your approved provider within fifteen (15) business days of receiving valid documentation, subject to funding availability, documentation review, and internal approval. Funds are never issued directly to applicants.

Are You a Licensed Provider? Join Our Vetted Network

SBHF partners with licensed mental health professionals who have demonstrated experience working with our served populations. Joining the network allows you to receive direct scholarship disbursements on behalf of eligible clients.

 Clinical Qualifications We Look For

Specializations:

  • PTSD and Complex PTSD
  • Traumatic Brain Injury (TBI)
  • Moral Injury
  • Sexual Violence Trauma
  • Secondary Trauma and First Responder Trauma

Diagnosis and Clinical Focus:

  • PTSD / Complex PTSD
  • Traumatic Brain Injury (TBI)
  • Acute Stress Disorder
  • Anxiety / Depression / Trauma-Induced Disorder

Population Experience:

  • Military / Veterans
  • First Responders
  • Victims of Violent Crime
  • Survivors of Sexual Violence

 Administrative Agreement

Providers accepted into the SBHF vetted network agree to the following:

  • We agree to the SBHF Third-Party Disbursement Policy
  • We agree to provide a W-9 and formal invoices
  • We are willing to provide brief progress summaries with patient consent

Disbursement Requirements:

  • Submit a completed W-9
  • Submit a formal invoice including recipient name and service dates
  • Return unused funds if services are not completed

Modalities offered are documented as part of the provider application, allowing SBHF to match the right provider to each client’s clinical needs.
